Troop 42 Equipment Maintenance & Care

Medical Forms

Everyone in Troop 42 must complete and submit these each year
  1. Annual Health and Medical Record form Parts A, B, & C
    • Forms must be signed and dated
    • Physician and Parent signatures are required on Part B if prescription drugs are required.
  2. Troop 42  Troop Medications Permission Form
  3. Copy of BOTH sides of the Scout’s Health Insurance Card

Notes:

  1. All health forms are valid for one year.
  2. Please indicate “Any Troop 42 authorized adult” under the Adult Authorized to take youth to and from event section in the Annual Health and Medical Record form Part A.  This significantly improves our ability to coordinate car pooling for events such as weekend camping and summer camp.
